Location: Council Bluffs, IADescription: We are currently sourcing for a Home Hospice RN Case Manager. Perm PlacementOverview: We’re excited to train new RN’s that are passionate about hospice care. The position does require travel. Case load is 12 patients in a 60-mile radius, seeing on average 4-6 per day.Some to are LTC facilities and some to the patient’s residence. Mileage is reimbursed. Scrubs will be provided. Job Description: The RN Case Manager is the “hub of the wheel” to manage all areas of the patient care. Will collaborate with the Social Workers, CNA’s, Chaplin and Music and Massage Therapists. Will do pain and symptom management. The average length of stay in hospice care is 6 months. Where the patient calls “home” will be either long term facility or residence. Case load is 12 patients in a 60-mile radius, seeing on average 4-6 per day. Hours:Monday to Friday 8 AM to 5 PM with weekend and holiday rotations. There is always a need to be ” On Call” in Hospice. When working a Saturday and Sunday they will have the Thursday off prior to the weekend or Tuesday off after the weekend.Qualifications: A passion for hospice care and the ability to build relationships with both patients and their family members Current licensure and CPR certification. The ability to work efficiently with little direct supervision. A valid driver’s license and an insured automobile that is in good working order. Acute care experience in an institutional setting or home care experience is preferred, but not required. What works?
