Social Workers providean important role within the hospital by assistingpatients and families with personal and environmental problems which relate tomental illness or which interfere with their obtaining maximum benefits fromavailable services.
The ideal candidate will have an LCSW and:
Provide/oversee psychosocialevaluation/consultation/referral and triage of assigned patients.
Conduct clinical assessments ofpatient/family/total situation to establish intervention strategies.
Develop resources such aseducational programs for community agencies or hospital or ambulatorypersonnel.
Facilitate patient care conferenceswith other team members to review patient’s goals of care, provide psychosocialsupport, assist with advance care planning and ensure proper referrals are madeto community resources.
Ensure timely and effective transferbetween facilities and nursing units; also facilitate transfer to appropriatelevel of care in order to ensure quality care, maximum reimbursement, andminimizing cost/day.
Conduct or attend discharge planningconferences with interdisciplinary healthcare team members.
Supervise other members of thesocial work team in providing the same level of discharge planning
Facilitate and direct peer groupsupervision with interdisciplinary clinical team.
Provide and/or coordinate in-serviceeducation and training for clinical staff.
Coordinate the education andtraining for social work interns.
This is a Full-Time, 8 hour day,varied schedule. This position is a combination of administrative duties and patientcare. $3000 BONUS
Post-MSW Palliative Care Certification and/or Oncology Social Work Certification.
