Our understanding of aging related issues enables our team members to best serve the interests and needs of our clients and family members.Working with a multi disciplinary team of professionals, under the supervision of the Clinical Care Management Lead, and in partnership with an assigned care manager, supports the provision of comprehensive care management services, while continuing to develop the knowledge base and skills required to take on progressively more responsibility within the Care Management service area.

A successful candidate is passionate about working with older adults and adults with disabilities, is a tenacious and creative problem-solver, with a commitment to learning and growing in their knowledge of best practices in Aging Life Care and Sage’s Core Competencies for provision of Care Management services.Key Areas of Responsibility and AccountabilityAcquiring Professional-Level Experience and Knowledge in the Field of Aging Life Care, and Developing High Level Skills in the Provision of Care ManagementBased on agreed-upon standards and timeline outlined in Sage’s Care Management Associate Offer Letter, and further specified in the Care Management Training Manual, the Care Management Associate learns an array of skills and practices, with the goal of assuming progressively greater levels of responsibility and autonomy in handling Care Management cases.Knowledge and skills include, but are not limited to the following:Conducting holistic, client centered, comprehensive in-home assessmentsDrafting written summaries of findings and recommendations in language that is clear and comprehensible to clients’ responsible parties, and that is useful in formulating or updating a client’s care planDeveloping clear and concise, goal-oriented care plans upon completion of the initial client assessment to be shared with the client and/or responsible party for review and developing a follow up plan.Implementing all aspects of the care plan, including following through on action items, identifying barriers to achievement of goals, and modifying Care Plans as needed.Navigating family dynamics to maximize success in achieving client goals.Effectively communicating and collaborating with family members and outside professionals to advocate for clients’ needs.Developing extensive, up-to-date knowledge of community resources available to older adultsCare Management Support and Direct ServicesThe Care Management Associate provides direct services that support the work of the Care Managers.

Specific assignments and responsibilities not only support Care Managers, but also often provide opportunities for on-the-job learning and further skill development.

Responsibilities include, but are not limited to:Resource Identification: Researching the highest quality, well-vetted resources, goods and services for our clients, helping them to maintain their highest level functioning and quality of life.Special Projects: Partnering with Care Managers to perform research on assigned topics and providing a clear report back and proposed action plan.Partnering with Care Managers, as assigned, implements specific components of the care plan, as requested by the Care ManagerCommunicates regularly with care managers to assess and provide feedback on supplemental client needs and services to achieve best outcomes for their clientsProvides feedback to Care Managers on changes or concerns noted in communication and contact with clientsAdvocates for clients’ needs with professionals and community-based servicesCompletes required documentation of servicesOther Duties and Responsibilities:Providing on-call services as part of an on-call team rotation.Providing coverage of cases as needed and assignedActive participation in Care Management meetingsProviding billable services to clientsKnowledge, Skills, and Attributes:Demonstrated ability to learn new skills and a strong desire to acquire the experience and knowledge needed to provide high level, professional services to Care Management clientsExcellent listening and communication skills, both written and verbalResourceful in researching and making use of high quality local and other eldercare resourcesA sense of optimism about what’s possibleCommitted to working collaboratively with a multidisciplinary team; open to feedback and new perspectivesTenacious, resourceful, and creative problem-solverTakes responsibility for work assigned, seeking guidance when needed, and continuously pursues excellence.Able to maintain calm in the face of high emotion, and crisisAble to reflect and communicate clearly and logically in the face of complex issuesEffectively prioritizes work and manages time efficientlyImpeccable attention to detail and strong follow throughCommitted to maintaining honesty, integrity, transparency and professional ethicsProficient in Microsoft Office Suite, Google Drive, Google ChromeOther Qualifications:Bachelor’s degree in a human services or related field, required.

Masters level, preferred.Experience in community or facility based, client centered work, preferably in a healthcare related fieldKnowledge of eldercare resources preferredAvailability all weekdays and some evening/weekend on call rotationValid California driver’s license, insurance, personal transportationSage offers competitive salary + company bonus + significant annual increases.
