Due to the sudden increase in our growth and expansion, we are currently seeking qualified RN Case Manager’s to join our team!

We are in search of an experienced, full-time RN Case Manager who evaluates, implements, and establishes goals and objectives for hospice services that meet and promote the standards of quality and contribute to the total organization and philosophy. The RN Case Manager directs the care of his/her patient caseload, communicates with other team members, families, and assisted living and skilled nursing facility staff (if necessary).

GHC Hospice ranks much higher on the CAHPS Hospice Survey than the National Average.

This survey is conducted by a third-party firm and completed by family members of patients who have previously been on our hospice service.
