As the RN Case Manager, this person will be involved in hospital Case Management, Utilization Review, and Social Work.
The RN Case Manager will be responsible for staff assessment, daily operations, and will act as the liaison between Case Management, Coding, Compliance, Information Systems, and other departments.
To maximize success in this role, the RN Case Manager will demonstrate the ability to coordinate the integration of social services into patient care as appropriate and foster a collaborative work environment.
