Gallagher Home Health Services
The RN Case Manager (RN) is responsible for assessing the needs of the patient, planning and coordinating the patients care, and developing and following the Physicians Plan of Treatment.
The RN is responsible for ensuring that the plan agreed upon by the patient, family and physician ensures that the patient reaches the outcomes established by the ongoing assessment of the patients needs.
The RN will perform responsibilities within the States nurse practice act, and in compliance with the plan of care.
Responsibilities
Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered Handle case assignments, draft service plans, review case progress and determine case closure Help clients achieve wellness and autonomy Take the extra mile and interact with patients to keep track of their progress and to ensure satisfaction Promote quality and cost-effective interventions and outcomes Adhere to professional standards as outlined by protocols, rules and regulations
Requirements
Effective communication skills Excellent organisational and time management skills Familiarity with professional and technical emerging knowledge Problem solving skills and ability to multi-task Compassionate with teamwork skills Current license, certification or registration
