Practice Transformation Director

Upstream USA

This is a Full-time position in London, AR posted November 22, 2021.

Upstream partners with states to provide training and technical assistance to health centers, increasing equitable access to the full range of contraceptive options.

After launching our first statewide initiative in Delaware in 2014, we have since launched projects in Washington state, Massachusetts, North Carolina and Rhode Island.

By 2023, Upstream will have partnered with health centers that serve over one million women of reproductive age annually across these states.

Upstream is funded primarily through philanthropic individuals and foundations, and does not accept funding from pharmaceutical companies.

Upstream’s budget has grown from $1.7 million just four years ago to approximately $51 million in 2021.

Upstream’s work has also garnered attention from many leading publications including The New York Times , Washington Post , The Atlantic , CNN , Vox , Forbes , and Marie Claire .

Position Summary

Reporting to the VP of Practice Transformation, the Practice Transformation Director (PTD) oversees quality delivery/program implementation for multiple healthcare agencies by optimizing within set resources.

They lead and supervise a program delivery team that can include a Practice Transformation Lead, a Practice Transformation Associate and Practice Transformation Coordinators.

The PTD will recommend agency scope and required resources and will determine agency work plan and execution.

Other key responsibilities include evaluating cost to deliver our programs at assigned agencies, evaluating agency performance outcomes and sustainability, ensuring the Practice Transformation team’s engagement including the agency’s achievement of core capacities and healthcare agency satisfaction.

The PTD will have limited management responsibilities for a Monitoring , Evaluation and Learning Specialist in relation to our agency engagement work.

Essential Duties and Responsibilities

  • Accountable for driving outcomes at partner healthcare agencies by delivering high-quality and consistent Program components, optimized for resources allocated and budgets set by VP of Practice Transformation
  • Responsible for coaching, mentoring, and guiding Program delivery team on Program execution, relationship management, and practice facilitation
  • Scope technical assistance work to assign workstream ownership (if applicable)
  • Acts as primary point of cross-functional coordination between Program delivery teams and functional support leadership (MEL, Communications, Program Design, Subject Matter Experts, etc)
  • Owns relationship with Agency Executive Sponsor in small to mid-sized agencies, acts as backup for relationship if there is a change in PTL, acts as resource for issues Agency raises about PTL
  • Responsible for key stakeholder management, as applicable, including precepting relationships and segment-specific stakeholders such as the MassLeague
  • Facilitates sharing of key learnings from program delivery with Program Design team for continuous learning
  • Collaborates with our Partnership and Community Engagement team to accurately scope healthcare agency work while providing a seamless end-to-end experience from negotiation through graduation
  • Support internal continuous learning and cross-functional efficiency: e.g.

    Policy work, Impact stories to Communications, etc.

Required Education, Experience, Knowledge, Skills and Ability

  • 10+ years of experience within health care setting with demonstrated supervisory responsibilities
  • Demonstrates deep knowledge or experience in sexual and reproductive healthcare or primary care best known practice and technical assistance program expertise and track record of commitment to continuous program improvement
  • Experience embedding evidence based clinical practices in primary care preferred
  • Understanding of primary care services delivery preferred
  • Excellent relationship management skills
  • Strong coaching and mentoring skills to develop direct reports and peers on quality Program implementation
  • Prior healthcare services delivery or systems experience (especially in primary care settings)
  • Decision-maker
  • Possesses strong listening skills, training in motivational interviewing a plus
  • Ability to be an advocate for agency facing staff
  • Innovative with the ability to evaluate outcomes and to reflect in order to course correct as needed.
  • Bachelors’ and/or Masters’ degree in related field or combination of applicable education and work experience.
  • Able to attend work related in-person meetings and functions as needed (Upstream employees are required to be fully vaccinated to attend in-person work-related activities.
