OUR AGENCY IS GROWING!!!WE HAVE A CASE MANAGER OPENING AT OUR HAMMOND LOCATION*Position is 30 hours per week.Hours are Monday through Friday 9:00 a.M. 3:00 p.M. However, some evening and weekend work may be required, but will not be often.Minimum QualificationsA Bachelor’s Degree in social work is preferred but will consider a combination of post-secondary education plus direct case management experience in youth services field. 2+ years’ experience in direct service programming. Demonstrated success engaging systems and/or individuals. Knowledge of local and national education reform and workforce development strategies. Ability to provide excellent customer service. Ability to design and implement systems necessary to gather, maintain and analyze statistical data and generate reports. Relies on experience and judgment to plan and accomplish goals. Performs a variety of tasks. Computer capability with Microsoft Office, Excel, Publisher, and PowerPoint.Required Skills and AbilitiesCommitted to the success of the program. Proficiency in Microsoft Office; Ability to work with audiences with a variety of cultural, ethnic, and educational backgrounds. Ability to present information effectively both orally and in writing to intervals and various groups. Flexibility in scheduling, able to work weekends and evenings as needed. Flexible, reliable, friendly, and conscientious. Able to work with both youth and adults with a respectful and non-judgmental attitude. Capable of being appropriately responsive to challenging and/or crisis situations. Practice good verbal and written communication and organization skills. Compliance with confidentiality and mandatory reporting of child abuse and neglect. Excellent organizational, communication, writing and speaking skills. Must have relationship development experience with youth and adult populations. Ability to form appropriate reintegration relationships. Relates well in multicultural environments. Ability to collect meaningful data and draw conclusions. Reliable transportation, auto insurance and valid LA driver’s license required.ResponsibilitiesAssists youth with IIPsAnalyze information from interviews, educational and medical records, consultation with other professionals, and diagnostic evaluations, in order to assess clients’ abilities, needs, and eligibility for servicesAssess client needs and design and implement rehabilitation programs that may include personal and vocational counseling, training, and job placementConduct client home visits, as neededCollaborate with clients’ families to implement rehabilitation plans that include behavioral, residential, social, and/or employment goalsDevelop and maintain relationships with community referral sources such as schools and community groupsMaintain close contact with clients during job training and placements, in order to resolve problems and evaluate placement adequacyMonitor and record clients’ progress in order to ensure that goals and objectives are metPrepare and maintain records and case files, including documentation such as clients’ personal and eligibility information, services provided, narratives of client contacts, and relevant correspondenceAdminister satisfaction surveysCollaborate with community agencies to establish facilities and programs to assist persons with disabilitiesParticipate in job development and placement programs, contacting prospective employers, placing clients in jobs, and evaluating the success of placementsPerform other duties as assigned by the Program Manager and/or Project DirectorJob Type: Part-timePay: From $15.50 per hourSchedule:Monday to FridayCOVID-19 considerations:All employees are required to wear masks in the office and when engaging with the community.Education:Bachelor’s (Preferred)Experience:Case management: 2 years (Preferred)Work Location: One location
