Case Managers

social worker counseling Case manager Lead to join our team in San Antonio, TX supporting a humanitarian assistance project

Responsibilities

• Directs, coordinates, and oversees case management to assure quality cost effectiveness of high-risk cases
• Evaluates outcome of patient care
• May include prospective and retrospective review that includes, but is not limited to, prior authorization, determining the appropriate level of care and utilization of services, concurrent review, retrospective review, developing discharge plans, and ensuring quality cost effectiveness
• May arrange referrals, consultations, therapeutic services, and confer with other specialists on course of care and treatment

Qualifications

• Bachelors in related discipline or directly relevant work experience
• 5-7 years of relevant work experience with Social work and counseling
• Strong written and verbal communication skills
• Bilingual Spanish and English
• Must be able to work independently or within a team

Job Requirements:

Preferred Qualifications

• Certified Case Manager preferred
• 10 or more years of clinical experience with social work or counseling
