Case Manager

Mat-su Health Services

This is a Full-time position in Wasilla, AK posted February 6, 2022.

Position Summary

Under the general supervision of the Rehabilitation Services Coordinator or Designee, the Case Manager provides outreach and case finding, case management, assessment and planning, and case coordination between client, family and community resources.

Provides monitoring and advocacy for individuals who suffer from severe and persistent mental illness.

Works to develop and facilitate rehabilitation activities that include skills development, and recreational opportunities.

This position requires compliance with MSHS’ compliance standards, including the Code of Conduct, Compliance Program, and policies and procedures.

Such compliance will be an element considered as part of the regular performance evaluation.
Maintains regular contact with psychiatric hospitals, jails, shelters, and other locations to identify and offer services and assist qualifying clients/families to obtain benefits and connect to services.
Maintains accurate client services records and distributes completed information to service providers and medical records within the established time frame.
Reviews treatment plan goals and client progress as required by the current Division of Behavioral Health regulations.
Completes prior authorizations as required, monitors progress, and provides follow-up as necessary.
Provides and participates in interdisciplinary quality assurance, utilization review, treatment planning, treatment assessments, functional assessments, and crisis intervention.
6.) Works within the productivity standards as currently established by the Agency.
7.) Attends supervision, case managers’ meetings, and training as required.

Follows guidelines of medical necessity, clinically justifiable, and active treatment when forming goals and interventions.
Assists consumers in obtaining and maintaining entitlements and employment, and refers consumers to community resources, supports, and services.
Monitors and coordinates client services and the service delivery system to assure the consumer receives appropriate and necessary care.
Provides direct services to include individual and group skills development.

Researches and develops curriculum and prepares class materials to teach skills building classes.
Advocates on behalf of the consumer to secure needed resources, supports, and services.
Remains familiar with and adheres to published Agency policies and departmental requirements.
Nonessential Functions

Provides access to community resources for clients as scheduled utilizing Agency vehicles.
Provides education about mental health and services to outside agencies and organizations as requested.
Actively works to assure implementation of the values and principles of psychosocial rehabilitation.
Other position/program related duties as assigned.
Working Conditions

Work is performed primarily inside a heated, ventilated, and well-lighted environment.
Job is considered low-risk for hazard exposure.

Position Requirements:

Education and Experience
BA/BS degree preferred in Social Work, Psychology, Human Services, or related field with some work experience.

5 years of relevant work experience may be substituted for education.

Education & relevant work experience equivalent to 5 years may be considered.
Knowledge Skills and Abilities
Demonstrated ability to communicate effectively, both orally and in writing.
Ability to independently plan and schedule program activities.
Knowledge of standard office equipment including personal computer and standard word-processing programs experience.
General
Clear background check (subject to DHSS fingerprinted background check)
Current driver’s license; clear driving record
Current tuberculosis screening test result; Mat-Su Health Services requires all staff to be vaccinated against Covid-19 consistent with CMS mandates.
Current First Aid and CPR certifications.

CPI (Nonviolent Crisis Intervention) certifications must be obtained when offered through the agency; may substitute current MANDT certificate for CPI.
