Description of JobPrimary Job Duties+ Provide transportation and assist with coordination of transports for all residents at the Colorado Veterans Community Living Center Homelake.+ Safely transports residents to and from medical appointments.+ Coordinates transportation between nursing home and domiciliary.+ Completes and posts weekly schedules of transports.+ Communicates with other departments if back-up transports are needed.+ Maintain cleanliness of all facility vehicles.+ Maintain and stock necessary supplies in all facility vehicles (first aid kit, personal supplies for residents, oxygen, as needed).+ Coordinates maintenance of vehicles with physical plant manager (oil changes, tire wear, etc.). + Reports damage to vehicles as they occur. + Maintains record of mileage and maintenance of all facility vehicles.+ Performs activities of daily living (ADL) during transportation, if required.+ Assists with moving in of new residents, lifting, moving furniture, cleaning of cottages and disinfecting toilets, sinks and showers.+ Washes floors, vacuums, trash removal, dust, general maintenance (light bulbs, hanging pictures, etc.)+ Nursing Home and Domiciliary floor coverage includes performing basic nursing care, assist with providing nutrition for residents that need assistance, dressing, transferring, ambulating, positioning, lifting, range of motion, toileting, skin care, and all other duties as assigned.+ Training and mandatory in-services for facility, including CPR and First Aid.+ Attend meetings as scheduled.+ Reports any emergency situations immediately to Domiciliary or Nursing Home nurse.+ Other duties as assignedMinimum Qualifications, Substitutions, Conditions of Employment & Appeal RightsMinimum Qualifications:NonePreferred Qualifications:There are no education or experience requirements for entry into positions in this job class. However, the ideal candidate will have a HS diploma or GED and at least 1 year experience related to the position duties listed above.Must possess a current and active Certified Nurse Aide license obtained from the Colorado Department of Regulatory Agencies or be willing to obtain a license within four months of hire date.**Please attach CNA license to this application, if possible.**Conditions of Employment:+ Full Background – Name check, TRAILS and Fingerprinting+ Colorado Driver’s License+ Must have or be willing to obtain an active Client Care Aide/Certified Nurse Aid (CNA) license within four months of employment.+ Shift work: evenings, weekends, holidays+ Ability to climb, stoop, kneel, crouch, crawl, and reach throughout an eight hour work shift+ Ability to use, handle and dispose of hazardous waste & assist in emergency clean-up+ Ability to handle invasive medical devices and/or equipment.+ May involve travel, including transportation of residents outside the San Luis Valley for ongoing medical appointments, transporting items, going to stores for supplies, or attending trainings.+ This position is considered essential services, which means the employee is required to report without delay or interruption to provide essential or emergency services to ensure health, safety, and welfare to residents.+ Former State employees who were disciplinarily terminated or resigned in lieu of termination must disclose the information on the application and provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position.
