Youth Transition Specialist

Ability 360

This is a Full-time position in Phoenix, AZ posted October 21, 2021.

Position Summary: Established in 1981, Ability360 is a 501(c)(3) nonprofit organization providing independent living services for persons with disabilities.

Ability360 provides a variety of services to ensure that students with disabilities make a successful transition to adulthood.

These services help students with disabilities train for employment, continue their education, or secure employment upon transition out of high school.

Program Service: Ability360 delivers high-quality Pre-Employment Transition Services (Pre-ETS) that include services, activities, and guidance in the following categories: Self-Advocacy, Workplace Readiness Training (Independent Living Skills), Counseling on Post-Secondary Education, Job Exploration, and Work-Based Learning.

Our team provides guidance, support, and career development opportunities to students with disabilities ages 14-22.

Individuals must be driven by a desire to improve outcomes for youth and young adults by working with youth one-on-one and in group settings providing instruction and support for participants to reach educational, employment, and independent living goals.

Required Education and Qualifications:

  • Bachelor degree in education, human services or related field.
  • Experience working with youth with disabilities in a consumer-driven environment.
  • Outstanding communication and writing skills
  • Strong decision making and problem-solving skills.
  • Excellent organizational skills.
  • Ability to take initiative and work with minimal supervision.
  • Experience developing and implementing programs.
  • Ability to effectively establish and maintain professional relationships with consumers, staff, and community partners.
  • Proficient in Microsoft Word, Outlook, and Excel
  • Pass Criminal Background Check
  • Bi-lingual highly recommended

Essential Responsibilities / Accountabilities:

  • Small and large group facilitation both in-person and virtual environments.
  • Complete in-person and virtual intakes for all consumers.
  • Work directly with youth to develop independent living goals and actions steps to accomplish those goals.
  • Maintain accurate and current consumer service records.
  • Conduct outreach and coordinate community engagement activities.
  • Develop partner relationships with other organizational programs.
  • Assist in the marketing of the program, including meeting with community partners and school administrators.
  • Maintain cooperative working relationships with internal and external partners
  • Other duties as assigned

Position may require local travel by car, and work in and outside the office.

Position Type/Expected Hours of Work:

This is a full-time position.

Work schedule requires some flexibility and will include some evening and weekend work.
