Social Work Care Coordinator
Seattle, WA (local remote)
Our national managed care company’s Health Plan is hiring a licensed program coordinator care coordinator for our expanding social worker team.
This Program Coordinator will conduct screening and risk assessment interventions per program guidelines.
Assist with non medical aspects of the member’s care, including referrals to community resources

Position is full time right to hire.
Pay up to 24HR.
Will be remote telephonic social worker setting.
Requirements:
2+ years of social work experience in an acute care or community setting.
