DEPARTMENT SUMMARY: The St. Charles Medical Group administration department contains the leadership team of the St. Charles Medical Group and services that help support the operations of the medical group. This includes the ambulatory quality team, daily operations, nursing excellence and others. The department exists to provide our frontline caregivers with the support and tools they need to serve our patients and community in a high quality, timely and efficient way.

POSITION OVERVIEW: The Triage Registered Nurse provides quality patient and family centered care as a member of the health care team in accordance with SCHS mission, philosophy, policies, and procedures. This position applies standards for professional nursing practice, clinical care and triage assessment to enhance patient safety and experience. The Triage RN acts as liaison between patients and clinic professional staff by facilitating patient triage from within a centralized call center department, remotely, or on site. This position does not directly supervise any other caregivers.

ESSENTIAL FUNCTIONS AND DUTIES:

Maintains and demonstrates an in-depth knowledge of nursing principles, practices, standards, and techniques, and applies this knowledge in accordance with SCHS policies and work instructions and within the laws and regulations governing nursing practice in the State of Oregon.

Participates in patient triage virtually or on site, assessing the patient’s condition and needs using critical thinking and clinical judgment: set outcomes; implements appropriate nursing actions and collaborates with other disciplines to meet the patient’s/family’s physical, emotional, spiritual, social, and intellectual needs; and evaluates patient’s progress.

Participates in patient care as a member of the local and extended care team, which exists to support and enhance patient wellness and recovery. Creates a healing environment that supports all aspects of the care environment and the wholeness of each individual, patient and caregiver.

Provides a therapeutic presence in service to others by purposefully responding to the needs of patients in a caring way, including introducing oneself and explaining role in patient’s care, asking the patient his or her preferred name, demonstrates active listening, and communicating effectively and appropriately through touch, eye contact, etc.

Provides and maintains a safe environment for caregivers, patients and guests.

Documents all patient care with proficiency in compliance with hospital policies, procedures and regulatory agencies.

Supports the vision, mission and values of the organization in all respects.

Supports Value Improvement Practice (VIP- Lean) principles of continuous improvement with energy and enthusiasm, functioning as a champion of change.

Provides and maintains a safe environment for caregivers, patients and guests.

EDUCATION:

Required: As required by licensure.

Preferred: BSN degree.

LICENSURE/CERTIFICATION/REGISTRATION:

Required: Current Oregon RN license. BLS/CPR.

Preferred: ACLS preferred.

EXPERIENCE:

Required: Minimum of two (2) years nursing experience preferable in an ambulatory setting.

Preferred: At least two years of ambulatory clinical RN experience.

PERSONAL PROTECTIVE EQUIPMENT:

Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.

PHYSICAL REQUIREMENTS:

Continually (75% or more): Use of clear and audible speaking voice and the ability to hear normal speech level.

Frequently (50%): Sitting, standing, walking, lifting 1-10 pounds, keyboard operation.

Occasionally (25%): Bending, climbing stairs, reaching overhead, carrying/pushing or pulling 1-10 pounds, grasping/squeezing.

Rarely (10%): Stooping/kneeling/crouching, lifting, carrying, pushing or pulling 11-15 pounds, operation of a motor vehicle.

Never (0%): Climbing ladder/step-stool, lifting/carrying/pushing or pulling 25-50 pounds, ability to hear whispered speech level.

Never (0%): Heat, cold, wet/slippery area, noise, dust, vibration, chemical solution, uneven surface.
