Referral Coordinator

RI Primary Care Physicians Corporation

This is a Contract position in Cranston, RI posted November 27, 2021.

JOB SUMMARY

The Referral Coordinator provides support to patients as part of Rhode Island Primary Care’s (RIPCPC) interdisciplinary care team. The Referral Coordinator will accurately and thoroughly document all assigned referrals within the Electronic Health Record (Epic) during every step of the referral process from initiation to release back to the practice after appointment is scheduled. The Referral Coordinator works with adult and pediatric primary care practices to respond to their patients’ referral needs.

Effective October 1, 2021 all RIPCPC employees must be fully vaccinated for SARS CoV2 (COVID-19).

ESSENTIAL JOB FUNCTIONS

  • Assist provider practices with scheduling specialist appointments for patients with all insurance types (Blue Cross, United Health, Medicare, etc.)
  • Ensure accurate and thorough documentation of referrals within Epic (E.H.R.) during every step of the referral process from initiation to closure. This documentation will allow all Epic users to see the current status of any open referral.
  • Verify patients’ current insurance coverage.
  • Work collaboratively with office practice staff, physicians, and other providers.
  • Work collaboratively with RIPCPC staff including, but not limited to, nurse care managers (NCM), pharmacists (RPh), patient care coordinators (PCC), and quality improvement specialist (QIS) team members.
  • Actively participate in staff meetings as scheduled and education sessions as needed.

QUALIFICATIONS

Minimum Education and Experience

  • 1-2 years’ experience working in a health care related field, with direct interaction with clinical staff preferred.
  • Experience with documenting in an electronic health record.
  • Effective oral and written communication skills.
  • Skilled in patient-engagement and customer support.
  • Skilled in telephonic patient interactions.
  • Basic understanding of health care system and how to assist patients in navigation.
  • Detail orientation and ability to juggle multiple tasks.
  • Highly self-motivated.
  • Team focus required.
  • Ability to manage change in a positive manner required.
  • High school diploma required.

Preferred Education, Additional Qualifications and Experience

  • Trained on Epic Electronic Health Record (E.H.R.).
  • Bilingual – Spanish preferred.
  • Experience working in interdisciplinary teams.
  • Knowledge of specialty care resources across the state.
