– Amador County works in the Sutter Amador hospital emergency room and the WellSpace Health Amador Community Health Center and acts as a liaison identifying and assisting eligible Medi-Cal patients and connecting them to appropriate Primary Care services in the community.
The ED Navigator will also access community referral resources for patients, link eligible Medi-Cal patients to WellSpace Health Health Centers and programs, and various other community medical primary health care providers per patient preference.
The ED Navigator is expected to display excellent customer service and professionalism at all times in either work location.Required QualificationsHigh School Diploma required;AA degree or AOD education preferred;OR work experience sufficient to successfully perform the essential duties of the job.Ability to work assigned work hours during peak hospital usage hours as scheduled.
Ability to work overtime as assigned or as needed when approved prior by Manager.Ability to write progress notes and maintain detailed, statistical records.Report weekly on number of referrals, appointments made and links to community resources to designated team member.Demonstrated commitment to the provision of services for the underserved and sensitivity working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic, and cultural origins; disabled, homeless, substance users, HIV (AIDS) infected, and/or physiologically impaired.Communicate clearly and concisely, both verbally and in writing and demonstrate a high level of listening skills.Must be able to maintain appropriate boundaries with clients.Knowledge of substance abuse issues and treatment available in the community.Knowledge of community resources.Ability to establish and maintain positive, professional relationships.Able to function effectively in a fast paced environment.Current CPR and first aid certificate or willingness to be trained required within first (90) ninety days of employment.Ability to abide by standards of professional ethics and maintain HIPAA rules of confidentiality.Computer (PC) literate in Microsoft Word, Excel, Powerpoint, Outlook email and database software.Data entry into practice management system with 95% or higher accuracy.Adherence to infection control procedures, including but not limited to, standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking and social distancing.Assist and support the Center/Department/Program to meet standards of High Reliability.Must have valid driver’s license and insurable driving record.
May be required to drive agency vehicle and transport patients.Must have a reliable vehicle, and be willing to transport patients in personal vehicle as needed or determined by T3/EDN Program Manager.Essential ResponsibilitiesRepresent WellSpace Health professionally and with excellent customer service when meeting with patients presenting in the emergency room department who lack a primary care provider, or need assistance accessing a primary care provider, or need community health resources referral information.Transport patients to health care settings either in agency vehicle or in personal vehicle, as needed or determined by T3/EDN Program Manager.Coordinate linkage to primary care providers and supports by scheduling appointments and providing telephonic follow-up to ensure patient has received the health care that they needed.Refer and schedule eligible patients with appointments with health care providers at WellSpace Health Health Centers or other primary care provider, as is appropriate.Consistent, reliable attendance for assigned work hours is mandatory for this position.Work with various hospital staff to coordinate referrals for eligible patients; represent WellSpace Health professionally and with excellent customer service to hospital staff.Attend weekly rounds with Sutter staff at the hospital to communicate current patient needs and foster on-going consistent communication between WellSpace Health and Sutter.Keep accurate records for reporting to various funding and inspecting agencies.Maintain extensive knowledge of WellSpace Health programs and key staff, especially in Amador County.Attend regular meetings with WellSpace Health management and team.
