SUMMARY The Patient Care Coordinator delivers superior customer service while receiving and processing referrals initiated at the customer care center point of entry.
Performs all functions that facilitate the efficient processing of referrals and promotes overall patient satisfaction with RRH Home Health and Hospice Services.
Also responsible for the development, education, implementation and coordination of infusion home care plans.
This activity is largely done in facilities, preparing patients for their admission to or continuation of home care services upon their return to their places of residence.
STATUS: Full Time LOCATION: Rochester, NY DEPARTMENT: Home Care SCHEDULE: Day Shift, 40 hours ATTRIBUTES Valid NYS RN license and graduation from an accredited school of registered professional nursing with a minimum of 1 year of community health RN or related experience required and a Valid NYS drivers license and ability to travel.
A minimum of 2 years customer service experience required.
Maintains knowledge of insurance and reimbursement guidelines and changes.
Basic computer skills are necessary.
With training provided, must demonstrate the ability to learn and utilize new systems, computer applications and operating environments as necessary.
Flexibility to work a varied work schedule when needed to meet department needs.
RESPONSIBILITIES Referrals.
Screens all inquiries from referral sources (hospitals, physicians, clinics, patients/family members, etc.) and provides accurate and reliable information regarding the criteria for admission to home health care/hospice services.
Documentation.
Obtains necessary patient specific information at the time of referral to maximize the potential that patient will meet admission criteria.
Appropriately and accurately inputs admission data and completes referral and coordinates service requests with a focus on the patients care needs and to assure a timely admission to service.
Billing.
Obtains required billing information to process verification and billing authorization and generates physician orders.
Customer Service.
Answers calls and obtains pertinent information to quickly and accurately identify and assess callers needs and determine action steps to satisfy those needs or to resolve basic types of customer service inquiries.
Community Liaison.
Acts as a liaison with the medical community at large regarding the Rochester Regional Health Home Care and Hospice Infusion programs.
Patient Care Assistance.
Provides Assistance and support to Infusion Liaisons in coordinating infusion home care plans for patients and potential patients.
Policy Development.
Participates in the development and maintenance of current operational policies and procedures related to agency infusion programs.
