Crisis Preparation & Recovery
The Outpatient Counselor provides professional behavioral health services designed to decrease the impact of behavioral health disorders and other emotional disturbances.
The primary responsibility is to provide assessment and treatment; which requires the application of specialized knowledge in the area of behavioral health diagnosis and intervention.
Work is conducted on an independent basis but is regularly reviewed to ensure that caseload is properly managed, clients are progressing satisfactorily and treatments and interventions are done in accordance with policies, procedures and regulations.
The Tempe counseling site is eligible for the National Health Service Corps Loan Repayment Program.
Knowledge and Skills: Excellent oral and written skills to communicate in a professional manner with clients Strong clinical skills to diagnose and treat behavioral health issues Attention to detail and able to problem solve and work independently.
Ability to work within an electronic medical record system (EMR) Duties: Determines client service needs by assessing individual client’s level of functioning, behavioral health symptoms and history Determine diagnosis and treatment needs and develops individualized client treatment plans.
At each session, determines the degree of Danger to Self (DTS) behavior or intention of patient At each session, determines the degree of Danger to Others (DTO) behavior or intention of patient If needed, coordinates any calls to 911 or petitioning of client for court-ordered evaluation After each session completes progress note, including date and time, and places in chart Completes form/note with the correct diagnosis and billing code are used Documentation must be clinically appropriate when reviewed by CPR QA Documentation must be in compliance with CPR training and examples given If counseling continues beyond one visit, prepares treatment plan for client’s approval Reviews treatment plans with client and modifies if necessary.
Obtains signature Assists client if needed with follow-up and follow-through on referrals given Reviews treatment plan and completion of goals with client Completes discharge plan when clinically appropriate Other duties as assigned Qualifications and Education Requirements: Current, unrestricted license issued by the Arizona Board of Behavioral Health Examiners: LCSW (Licensed Clinical Social Worker), LMSW (Licensed Master of Social Worker), LPC (Licensed Professional Counselor), LAC (Licensed Associate Counselor), Ph.D (Doctor of Philosophy), Psy D (Doctor of Psychology).
Documented post-master’s experience for two (2) years, including experience within the past 24 months providing Outpatient counseling services, or an equivalent of experience, skills and education.

Background check, as required by state.
