This is a Full-time position in Wichita, KS posted September 20, 2021.
Seeking a compassionate, enthusiastic, goal orientated , and resourceful RN to manage the after hours care of our patients and families .
The On-Call Team responds to after hours phone calls promptly and appropriately , and makes patient visits to ensure the comfort and safety of clients and their caregivers.
Full Time On Call position
– 7 days on, 7 days off Hospice Experience Preferred

Responsibilities
Provide education to the patients/family members regarding the end of life process, plan of care and offers guidance with health care decisions.
Consult with the attending physician and Hospice Medical Director concerning changes in patient condition requiring order changes and/or changes to plan of care.
Coordinate hospice care by working with other team members as well as the patients/family members to ensure needs are met.
Conducts initial as well as on-going comprehensive assessments.
Preform evaluations for hospice care along with informational visits.
Requirements
Demonstration of strong communication skills Ability to work in fast paced environments Kansas Nursing License Strong organizational and self management skills Strong and compassionate customer service skills Valid driver’s license
