Full Time Hospice STNA

As a Hospice STNA, you have the special opportunity to provide one on one hospice and palliative care to terminally ill patients in their home.

In collaboration with a full hospice interdisciplinary team, your work will include the development and delivery of a patient specific care plan for the patient and caregivers.

Looking for a Hospice Caregiver to help in our Akron/Canton territory which includes Akron, Canton, Dover, and Millersburg.

Collaborate with the Hospice Team, patients and their families, and other members of the patient care team to coordinate, provide and maintain continuity of patient care.

Monitor, document and report progress of patient’s health and condition using required documents, reports and established timelines.

Carry out the plan of care according to state regulation and communicates changes to the Hospice Team.

For full time employees we offer full time benefits including medical insurance dental insurance vision insurance short and long term disability 15k free life insurance Job Requirements: Promote effective written/verbal communication Change simple dressing (non-sterile dressings only) and report changes in wound status Performing all day-to-day operations of the library: reference, cataloging, and management of the automated system, circulation, collection development and serials management Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered with appropriate completed/demonstrated skills competency Maintain medical records in accordance with HPC policies and procedures Visiting Physicians Association (VPA), with a focus on senior living facilities, community organizations, hospitals, the general public, and referral sources throughout designated areas Directed by state law and the client’s plan of care, reminds clients to take medication Providing hands-on personal patient care and maintaining a safe environment and observing appropriate infection control precautions Participate in Nurse LPN on
– call rotation/schedule in order to meet the needs of patients and families Meet the personal care needs of the patient and supportive care for the caregiver Provide adequate nutrition and fluid intake Evaluate patient’s functional status and home environment for hazards or barriers to more independent living Accept and perform other related duties and responsibilities Demonstrate basic care to the primary caregiver Assist adult and pediatric patients in comfort of one’s home Create a sanitary and orderly environment Provide normal range of motion and transfers/positioning Minimize illness and/or injury Provide direct nursing care to seriously or terminally ill patients in accordance with established policies, procedures and protocols of the healthcare organization Meet or exceed expectations in the delivery of services, care and pain/symptom management by Jobble
