  • What you can expect:

    As a Diabetes Care Navigator, you will support the health and wellness of patients living with diabetes accessing care at Howard Brown Health. You will support patients by providing patient-centered health education, advocacy, resource navigation, and referrals to other health and wellness services.

    How you will make an impact:

  • Complete needs assessments with Howard Brown Health patients who may benefit from assistance in accessing medications, testing and treatment supplies, and other relevant medical and social services related to diabetes
  • Provide information, education, and emotional support to patients in a compassionate, culturally sensitive, and appropriate manner.
  • Evaluate patients’ abilities to understand health plans and provide relevant health plan navigation education
  • Assess financial need of patients and facilitate patient treatment financial support; help with applications, especially as it relates to prescription assistance programs
  • Develop and maintain required records, reports, and statistical data
  • Maintain understanding of the most current scientific knowledge regarding the treatment and prevention of diabetes by participating in ongoing continuing education
  • Maintain the highest level of client confidentiality
  • Other duties as assigned
  • Qualifications

    What you will bring to Howard Brown Health:

    Minimum Qualifications:

  • High school diploma or GED with 3-4 years experience or Bachelor’s degree with 1-2 years experience
  • Experience working in a community-based medical/social services setting
  • Experience and desire to work with diverse communities including Lesbian, Gay, Bisexual, Transgender, and Queer communities
  • Preferred Qualifications:

  • Case management experience
  • Knowledge and experience working with clients living with diabetes and other chronic diseases
  • Bilingual-English/Spanish language proficiency
