Certified Caregiver/CNA for Palliative Bridge ProgramCertified caregiver or CNA with minimum 3 years working experience with elderly communityAlzheimer’s experience a plusHave good time management skillsHospice experience a plusHave reliable transportation and be willing to drive to many different locations during the dayHave Valid auto insuranceHave current Fingerprint clearance Have current CPR/First AidHave Current TBBe able to work M-F 8am-5pm (no weekends)Must be Trustworthy and honestJob duties include but are not limited toDriving to each patient’s house on a set schedule and/or calling patients to set visitation schedule.Taking notes on patients concerns/needs and documenting those concerns/needs in patient chart in WellSky, By end of week Friday.Report to families, facility staff and Providers after each visit, reporting any concerns/needs that are observed by you or reported to you by patient such as cuts, bruises, falls, etc.
Clocking in/out using QuickTime, using time wisely as to not waste company time/money and tracking your miles correctly each day.
Submitting time by end of day Friday.Be able recognize need for and to assist patient/patient families in setting up outside resources such as but not limited to, Meal on Wheels, ALTCS, HH, Hospice, etc.
Be a friendly non-judgmental listening ear Assist patient in activities such as going to bingo, going for a walk, puzzle, arts and craft, etc.
Recognize need and encourage patient’s hydration, nutrition, exercising.
Documenting all in patient chart.
Attend all Bridge In-service meetings.
