This individual is responsible for coordinating and scheduling all intake appointments.

He/she will conduct all assessments with the client and legal guardian and obtain all necessary consent forms.

The Assessor will submit all assessments and progress notes electronically within 24 hours of the assessment and all consents and paperwork to Intake Coordinator within 1 week of assessment.

The principal functions of the position identified shall not be considered as a complete description of all the work requirements and expectations that may be inherent in the position.

nn nn n nn nn Essential Functions: nn Responsible for determining client’s psychiatric diagnosis based on diagnostic criteria from DSM-IV-TR .

Performs administrative duties related to intake activities.

Perform initial assessments in accordance with Medicaid regulations and updates as applicable.

Other duties as assigned.

nn n nn nn Education and/ or Experience: nn nnGraduation from an accredited college or university with a minimum of a Master’s Degree in Mental Health, Social Work, Marriage and Family or related field.

Must be licensed in related area with a minimum of 3 years clinical experience.

nn Demonstrates proficiency and effectiveness in verbal and written communication Ability to work with culturally diverse groups and individuals in order to facilitate participation toward common goals and objectives Ability to work collaboratively and professionally with all community partnerships Strong organizational skills Working knowledge of Excel and Word computer programs Must successfully complete all criminal background screens annually
