Advanced Practice Provider – Primary Care NP/PA

Overview Bluestone Physician Services is a mobile primary care practice offering a unique and robust model of care; customizing care that is preventive, proactive and includes all members of the care team, patients and their families.

Join us on our team-based care as a Float Nurse Practitioner or Physician Assistant as we deliver on our promise to tailor primary care to geriatric patients within Assisted Living and Memory Care environments in Blaine, Maple Grove and surrounding north to northwest Twin Cities communities.

The Float Nurse Practitioner or Physician Assistant is responsible for providing primary care to the patients of Bluestone Physician Services.

This position will float between different provider teams to cover PTO, leaves, unplanned time off, new business start-up, etc.

Position Description Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care facilities Full-time providers care for patients Monday-Thursday; option for Fridays to be a clinical or an administrative day See approximately 12-16 patients per/day Develop and implement care plans appropriate to patients and their desires Clinical support from clinical assistants and care coordination through LPNs, RNs, and social workers Access to diagnostics such as labs and x-rays as needed Collaborate with community partners within home care, hospice, and other resources as needed Qualifications Graduate of an accredited Adult, Adult/Geriatric or Family Nurse Practitioner or Physician Assistant Education Program Board Certified/Board Prepared through AANP, ANCC or NCCPA Licensed or ability to obtain licensure within the state of Minnesota to practice Ability to obtain and /or maintain DEA certification for the State of Minnesota Proven ability to communicate effectively with strong verbal skills Excellent interpersonal and customer service skills Must have a valid driver’s license and minimum state-required liability auto insurance; local travel is required Must have received or be willing to receive COVID-19 vaccination by date of hire.
