Access to Care Specialist

This is a Contract position in Sitka, AK posted December 15, 2021.

Overview: Access to Care is AltaPointe’s access-to-care department and the single point of entry into the majority of AltaPointe’s programs and services.

CarePointe is responsible for coordinating consumer care throughout our continuum, which may include facilitating hospital admissions and outpatient appointments, verifying insurance coverage, making discharge arrangements following hospitalization and providing referrals and linkage to other community resources.

Through a brief telephone screening, trained staff members assess an individuals needs for care, triage any crisis that the caller may be going through, and determine the most appropriate level of care and service.

The team members’ have behavioral healthcare experience and training.

Their educational credentials include bachelor and graduate degrees in mental health disciplines or related fields.

Responsibilities: Access to Care Specialists screens requests for services to determine the problem(s), assess risk level, and prioritize the needs of the individual.

They provide crisis intervention and resolution, including symptomatic relief, de-escalation or resolution of the situation, and referrals to appropriate services.

They also facilitate admission to program services ensuring appropriateness of level of care through use of clinical criteria.Essential FunctionsScreens requests for services to determine the problem(s), assess risk level, and prioritize the needs of the individualFacilitates admission to AltaPointe program services ensuring appropriateness of level of care through use of clinical criteriaCompletes initial screenings and accurately populates EMR for potential consumers, including those requesting outpatient and hospital services.Assists with needed follow up services regarding consumer well-being, payor source information, rescheduling, etc.Provides community resources and referral information to callers and consumers.Provides crisis intervention and resolution, including symptomatic relief, de-escalation or resolution of the situation, and referrals to appropriate services, when indicatedAdheres to customer service standards and professional code of ethicsSeeks supervision and consultation as neededAccepts and employs suggestions for improvementActively works to enhance skillsDocuments in a timely fashion per AltaPointe policyDocuments appropriately, completely, and comprehensively in EMRDocuments in a clear and concise mannerAssists and treats consumers with care, dignity and compassionRespects consumers privacy and confidentialityIs pleasant and cooperative with othersPersonal values dont inhibit ability to relate and care for othersIs sensitive to the consumers needs, expectations and individual differencesIs gentle and calm to consumers and familiesActively participates in AltaPointe committees as requiredCompletes assigned tasks in a timely mannerFollows AltaPointe policies and procedures Qualifications: Bachelors degree in a mental health discipline or related field.

Must have clear communication and good customer service skills, including patience, attentiveness, the ability to use positive language; great work ethic and a willingness to do what needs to be done.
