Stable Families Supervising Case Worker, Fairfield County

New Reach

This is a Full-time position in Bridgeport, CT posted November 19, 2021.

Primary Purpose:

To better support the efforts of New Reach to expand prevention services, New Reach is seeking a full-time Lead Caseworker to provide person-centered case management services to households at risk of eviction.

Services are designed to address housing crisis, behavioral health, and sustainability needs for all households served.

The Lead Caseworker will also provide supervision to caseworkers who deliver eviction prevention services under the Stable Families and Homeless Prevention programs to support quality service delivery, holistic assessment, and implementation of best practices within these program models.

Essential Functions:
Oversee and manage all referrals to Prevention services, including pre-screening, introduction to program services, eligibility determination, and management of the waitlist
Engage, enroll, and provide case management services to households at risk of eviction, including but not limited to assessments, action plans, crisis intervention, housing advocacy, landlord/tenant negotiation, budgeting services, employment support, access to mainstream benefits (SSI/SSDI, TANF, SNAP, etc.) access to healthcare systems to address unmet needs, access to recovery supports, and sustainability services to promote stable housing
Maintain client records, both in file format and electronic to meet reporting requirements
Maintain strict confidentiality of client records, in accordance with the New Reach Privacy Practices policy
Mandated reporting of any suspected or witnessed abuse of children to Department of Children and Families and of elderly clients to Elderly Protective Services
Develop and maintain effective working relationships with key stakeholders, including but not limited to Park City Communities, CT Legal Services, community providers, community members, and property owners/managers
Supervision of staff engaging in prevention case management services through Stable Families Program and Homeless Prevention Program
Facilitate weekly collaborative meetings between CT Legal Services and Stable Families Program
Facilitate team meetings and partake in community meetings in the absence of the supervisor
Monitor and manage Stable Families Program reporting documents, including google docs and google forms
Assist with data analysis and reporting of Stable Families Program outcomes
Assist in program development and quality assurance activities as directed by supervisor
Supervise, orient and train shelter staff to ensure that job responsibilities are being performed in a quality manner, consistent with policy and enforce disciplinary actions when necessary.
Responsible for the hiring, verification of time worked, monitoring and approval of PTO time, and the annual performance evaluations of direct supervisees.
Regular schedule is Monday through Friday 8:30am to 5:00pm.

Must be able to flex schedule to work some evenings to meet the individual needs of clients.

Qualifications:
Master’s Degree in Social Work or related Human Services field
Highly motivated, creative with excellent interpersonal skills
Experience in the provision of social services, case management, and supervision
Strong ability to lead and empower others; previous experience in managing or overseeing program implementation/administration preferred
Knowledge of housing law, eviction law, and subsidized housing policies preferred
Excellent computer skills required
Valid driver’s license and access to reliable transportation with proof of insurance required
Bilingual/Bicultural preferred
