The Social Worker is responsible for coordinating the discharge planning process, participating in interdisciplinary team meetings and quality improvement activities, as well as counseling and meeting the satisfaction of the patients, residents, family members and significant others.Position Qualifications:The ideal candidate will have a Master’s Degree in Social Work as well as active licensure in NYS, with prior experience in a long-term care, rehabilitation or hospital setting. Prior experience working with the elderly population preferred. Additionally, strong applicants will possess strong communication skills, and an ability to work collaboratively across a variety of disciplines.
