Job Summary and Qualifications

Job Summary: The Social Worker provides services to meet the social, psychosocial, and safety needs of the residents in FELC. The Social Worker/Case Manager is responsible for admissions coordination and discharge planning of residents. The Social Worker/Case Manager supports all marketing ventures on behalf of Highlands-Cashiers Hospital and Fidelia Eckerd Nursing Center. The Social Worker/Case Manager must be able to work effectively with residents based on developmental level.

Education and training: B.S. in a Human Service Field, including but not limited to sociology, special education, rehabilitation counseling, psychology, and nursing.A. Prefer one year of Social Work/Case Management responsibility in a nursing home/ Hospital or other related medical facility.B. Special Skills: Must have a holistic understanding of the needs of the elderly. Ability to interpret CMS and Joint Commission standards of care and comply with same. Knowledge of the MDS assessment process for FELC residents and the Payment System’s for FELC. Typing and computer skills are desirable.

Located between the towns of Highlands and Cashiers, the hospital offers 24/7 emergency care, acute inpatient care, rehabilitation, as well as long-term care through Eckerd Living Center. Ambulatory services are also available, including therapy services and primary care. Highlands-Cashiers Hospital has more than 24 beds for acute care services and 80+ beds in the Eckerd Living Center.
