As a Social Worker in Case Management you will have the opportunity to use your knowledge and background to help our patient populations in profound ways. You will be joining a collaborative team that values extraordinary patient care and dedication to one another. In this role:

  • You will assist patients and families with personal and environmental difficulties which predispose illness or interfere with obtaining maximum medical benefits.
  • You will collaborate with physicians, case managers, and other healthcare team members in patient evaluation and treatment to further their understanding of significant social and emotional factors underlying the patient’s health problem.
  • You will help patient and family members through individual or group conferences to increase their desire and ability to understand, accept, and follow medical recommendations. 
  • You will provide services planned to restore patients to their optimum social and health capacities. 
  • You will utilize community resources to assist patients in resuming life in their community or to help them live within limits of disability.
  • You will complete psychosocial assessments and assist with discharge planning for patients.

 

The largest medical facility in the region, Eastern Idaho Regional Medical Center (EIRMC) includes a full-service hospital with 318 patient beds. It serves as the region’s healthcare hub, serving Southeast Idaho, Western Wyoming, Yellowstone National Park and parts of Montana.  EIRMC is home to a Level II Trauma Center, a Level 1 ICU (of only two in Idaho), and the only Burn Center in the state of Idaho. Our ER is the most experienced in the region. The hospital’s other specialty services include the region’s only Level III Neonatal Intensive Care Unit capable of caring for babies born as early as 22 weeks gestation; the region’s only Pediatric Intensive Unit, the largest heart program in the region, a Behavioral Health Center which is a74-bed Joint Commission accredited psychiatric hospital  and the region’s only Primary Stroke Center.

  • Masters of Social Work required.  Verification of successful completion of accredited programs.  
  • State of Idaho license in social work required.  
  • Ethics/continuing education training as required by Idaho state law.
  • Current American Red Cross or American Heart Association Basic Life Support Course (BLS or BCLS) Certification.
  • Skills and knowledge base necessary for appropriate social service intervention.
  • Familiarity with community resources needed to intervene appropriately.
  • Skills needed to exercise independent judgment and to problem solve accordingly.
  • Ability to apply social work skills to various ages and populations.
