Provide educational counseling on issues related to chronic illnesses or disabilities, substance abuse and mental health.Major Duties:Develops plan of care with specific goals, objectives, and interventions defining actions unique to each patient’s needsPerforms and documents patient/family complete psychosocial assessment as neededLeads the team on discharge planning process ensuring that each patient has a safe and appropriate discharge plan to achieve positive outcomesFormulates a plan of intervention acceptable to the patient, family, and health care teamUpdates and revises plan of care when necessary, to promote enhanced outcomesCollaborates with all members of the healthcare team to develop, manage, and communicate patient needs and dischargeDocuments all interventions in the patient medical record both timely and accuratelyProvides advocacy, assistance, support, counseling and crisis intervention to patients and familiesFunctions as a liaison role between the hospital and community in making community resources available to the patient and familyProvides educational counseling and supportive intervention relative to issues related to coping with transplant, chronic illness or disability, decision-making, problem solving, and the impact of illness on patient and family.Develops and maintains department or team supported programs, support groups, educational programs, peer support, volunteer programs and special events.Distributes resource materials to patients and families along with arranging community referrals as neededParticipates in educational programs, in-service meetings, performance improvement, policy and program development, and other committee meetings when requestedComply with HIPAA, local, state, and federal regulatory requirementsOther duties as assignedMinimum/Preferred Requirements:Bachelor’s in Social Work, Master’s in Social Work preferred.Licensed Master Social Work (LMSW) or (Licensed Clinical Social Worker) LCSW.Prefer 1-3 years of acute health care experience or Skilled Nursing CenterBasic Life Support (BLS) certification upon hireWorking in an acute care setting or SNF within the past 12 months as a Social Worker in the specialty area.Knowledge of government/community resources such as Medicare, Medicaid, long-term care, or any other applicable resources/services
