– Following KISD’s 226 Day Staff Calendar You Will Be Responsible For: Utilizes evidence based interventions to assist patients, families, and significant others in coping with the psychosocial aspects of illness and its treatment.
Assessment of mental, behavioral, and emotional disorders Applying DSM criterial leading to diagnosis Establishment of an appropriate treatment plan, including consultation when indicated Provision of individual psychotherapy, family therapy, couples therapy, and group therapy as indicated.
Intervention in urgent and emergent mental health problems Provide such other services as may be determined by the responsible physician within your scope of practice.
Develops treatment plans in collaboration with psychiatric providers.
Consults with physicians, patient, hospital staff, family members or significant others in a timely manner to facilitate resolution of identified barriers to treatment progress to ensure the continuity of care.
Maintains pertinent records of social work intervention, assessment, diagnostic impression and treatment for use in compiling accurate statistics.
Documents social work intervention in the medical record that reflects the current findings and status of the intervention at the time of the intervention.
Makes referrals to Federal, state, county and community agencies in a timely manner.
Develops working relationship with agencies to ensure appropriate service delivery to patients.
Assesses patient for behavioral health needs as needed.
Provides referrals for behavioral services as needed.
Provides provisional diagnoses for mental health issues according to the DSM-V as needed.
Provides referrals or consults with additional team members when identification of issues needs to be attended by the treatment team.
Maintains effective communication techniques with physicians, Nurse Practitioners, and staff members.
Knowledgeable of current Federal, state and other regulatory agencies in the areas of quality, performance, utilization and patient care.
Reports suspected abuse and neglect to Child Protective Services.
Exhibits empathy and acceptance of adolescent mood variance and attempts at independence.
Identifies social problems and financial limitations in patients that may contribute to discharge and treatment problems.
Identifies and provides info about community resources from which patients may benefit.
Ability to recognize, identify, and communicate potential abuse and neglect issues with patients and family members.
Effectively communicate with Adult Protective Services to ensure continuum of care during and after hospital discharge.
Maintains current Texas social work license and completes continuing education unit accordingly.
Remains current in BLS certification as required.
Develop and maintain internal and external contacts that enhance effective relationships for the hospital.
Develops a working relationship with community organizations to establish maximum resources available to patients.
Attend conferences/workshops pertaining to departmental activities.
Communicate effectively with team members of the Behavioral Health Center.
Compile timely resources and literature pertaining to clinic needs Initiates projects to improve the performance of the department and the care provided to patients.
Works independently by using critical thinking and creativity to incorporate evidence based social work frameworks.
Collaborates with other community agencies and organizations to improve knowledge and awareness of resources and services.

Specific tasks or responsibilities will be documented in the incumbent’s addendum as outlined by the incumbent’s immediate manager Qualifications What You Will Need: Requires graduation from an accredited Master’s Degree program in social work, counseling or a related field, and appropriate licensure as required by state law.
Licensed Social Worker, i.e.
MSW degree, LPC, LCSW, LMFT Requires knowledge of the principles, laws, and practices of social work in a psychiatric outpatient setting.
At least two (2) years of experience as a clinical social worker in a psychiatric hospital or outpatient clinic setting is preferred.
Experience should include individual, group, and family therapy and discharge planning in a therapeutic care setting.
Job Summary: The AdventHealth Medical Group, Licensed Clinical Social Work position is trained and licensed to treat clients with emotional, behavioral and mental health disorders.
Counseling sessions may be one-to-one, group therapy, or sessions involving family members.
As part of the counseling process, they may refer patients out to other specialists, conduct interviews, testing and risk assessment sessions with patients to evaluate their mental condition and formulate a treatment plan.
