All applicants will be considered for employment without attention to race, color, religion, sex, sexual orientation, gender identity, national origin, veteran or disability status”.ResponsibilitiesThe Manager of Care Management at Long Island Community Hospital is a licensed Social Worker who assists in ensuring daily operations are running appropriately; completes daily staff assignments and assists in schedule preparation, reports/tracks quality data per UM plan; attends rounds; has extensive knowledge and experience in discharge planning and throughput activities; collaborates with physicians, physician assistants, nurse practitioners and case management staff to ensure documentation is in compliance with regulatory rules and guidelines. This is a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual’s health needs through communication and available resources to promote quality cost-effective outcomes. The Manager educates and trains department staff in line with the clinical documentation initiative program. The Manager is vital member of the Utilization Committee and acts as a leadership representative of the Care Management Department as needed at other hospital committees and meetings.The Manager supervises staff, appraises performance involving observing and evaluating an employee’s work behavior and accomplishments with the purpose of measuring their job performance against expected performance. Performance evaluation can be a powerful tool for individual and organization growth because it helps us to develop staff and move us closer to achieving the Hospital’s goals. It also recognizes and rewards an employee’s contributions, motivates them to maximize their potential and when necessary lets the employee know when improvement in performance is needed in a particular aspect of his/her job.QualificationsCurrent NYS LMSW/LCSW License requiredCare Management Certification preferredMinimum of 10 years hospital clinical experience and 5 years of experience in discharge planningMust possess knowledge of City, State and Federal regulatory standards for hospital reimbursement, the utilization review process and discharge planning functions. Support and maintain compliance with Federal, State and Local organizational and certification rules and regulationsKnowledge of identification, assessment and intervention pertaining to high risk patients, care transitions/DC planning, and cost effective health care and reimbursement from public and private sectorsField Instructor CertificationCurrent screen certification and maintenance of competence through relevant ongoing continuing educationWorking knowledge/experience with hospital/care management software documenting electronically and compiling data from the electronic medical recordDemonstrates ability to take initiative, leadership ability, and the ability to effectively utilize brief change in the solution-oriented interventions to achieve desired outcomes using critical thinking and evidence based knowledgeCollects and analyze data and to prepare accurate reports
