Large managed care company looking for a Licensed Social Worker for a part-time (20 hrs/wk), contract position ending January 31, 2022.
As a Licensed Social Worker, you’ll be responsible and accountable for the coordination of patient services through an interdisciplinary approach, which provides clinical and psychosocial support through the continuum of care.
This is a high volume, customer service environment.
You’ll need to be efficient, productive and thorough dealing with patients, families and caregivers.
Strong computer and software navigation skills are critical.
You should also be strongly patient-focused and adaptable to changes.

PRIMARY RESPONSIBILITIES Continuum of care planning that integrates initial assessment of the patient with appropriate post-hospital discharge services.
Must demonstrate excellent communication and collaboration skills.
Ability to identify gaps and/or barriers in treatment planning.
Provide patient, family, and/or caregiver education.
Initiate and ensure completion of all necessary paperwork.
Provide referrals for community resources to patients/families/caregivers for ongoing support and treatment.
Report all relevant information/orders to providers and care team at patient’s next level of care.
Must be able to functionally coordinate and discharge plan for all age groups, including the unborn child through geriatric age.
Compliance with all regulatory requirements for Joint Commission Standards and CMS guidelines.
QUALIFICATIONS Master’s degree in Social Work or other related field 1 year experience in long-term care, home health, hospice, public health or assisted living 1 year of experience working with MS Word, Excel and Outlook Access to reliable transportation PRIMARY RESPONSIBILITIES LCSW Preferred 1 year of clinical or case management experience A background in managing populations with complex medical or behavioral needs Experience with electronic charting Experience with arranging community resources Prior field-based work experience ALL WORKERS ARE REQUIRED TO BE FULLY VACCINATED BY 11/15/21
