We are currently seeking two Social Workers to join the Geriatric Regional Assessment Team (GRAT) in Seattle!!!

SUMMARY:
The Geriatric Regional Assessment Team (GRAT) is a home-visiting team that assesses and develops interventions for individuals 55+ experiencing cognitive or behavioral issues escalating toward a crisis. The Social Worker responds to referent’s concerns, provides comprehensive assessments and develops interventions that increase client stability, supports independence, and prevents hospitalizations.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Initiates face to face home visits with client.
- Engages client to identify needs through in-depth assessment utilizing evidence-based tools, considering cognitive, behavioral, social and functional aspects.
- Investigates complex psycho-social situations with collateral contacts, environmental observations, and when possible, review of prior attempts to engage client.
- Develops and implements strengths based care plan with client.
- Mobilizes clients’ families and support circles to respond to client needs.
- Facilitates appropriate referrals to community programs, health care providers, APS and/or law enforcement. Advocates for service and provides necessary follow up.
- Actively seeks cultural guidance from diverse community members/agencies about how to best serve individual clients.
- Advances care plans, tracking progress, and closing contact when interventions are reasonably accomplished.
- Participates in GRAT team meetings to review assessments and interventions in case conference, to further strategize, collaborate, and confer with multidisciplinary team.
- Attend Assistance Services staff meetings, agency meetings, All-Staff meetings and trainings as appropriate or required.
- Demonstrates an awareness and eagerness to learn and enhance skills that promote equity and inclusion. Participates in events related to understanding institutional racism and other forms of oppression.
- Ensure areas of responsibility operate within the policies and procedures of Sound Generations and applicable governing agencies.
- Assist supervisor with program planning and operations.
- Assume and effectively discharge other related duties as assigned.
RELATIONSHIPS:
- Supervised by the GRAT Supervisor.
- Works closely with GRAT and Assistance Services team members.
- Develops and maintains good relationships with clients, staff, volunteers, and persons utilizing the same facilities and with social and governmental agencies and other organizations serving older adults.
QUALIFICATIONS:
EDUCATION & EXPERIENCE:
- Master’s degree in social work, psychology, counseling, or related field is required.
- WA State Department of Health clinical social work licensure (LICSW) or mental health counseling licensure (LMHC). Will consider those working toward licensure. CDP licensure and experience preferred.
- Experience performing psychosocial, psychiatric, and substance use assessments, and developing interventions relevant to risk, with older adult population.
- Minimum five years of professional experience working with older adults and/or individuals with cognitive or behavior issues, or comparable human services experience.
- Geriatric Mental Health Certification or the ability to obtain certification within 18 months of hire . Sound Generations will support the cost of these trainings.
SKILLS & ABILITIES:
- Ability to independently structure work tasks, and autonomously make decisions, sometimes in immediate safety situations.
- Valid WA State Drivers’ License and access to insured vehicle in order to travel to sites, community meetings and other locations in a timely manner. Familiarity with King County is a plus.
- Excellent interpersonal skills; a cooperative team player, able to develop productive relationships with colleagues, external collaborators, and aging population.
- Ability to maintain records of activities and contacts according to established GRAT procedures.
- Ability to use laptop and cell phone, as issued by Sound Generations.
