Case Worker II

Social Worker/Care Manager II San Mateo, CA $30-$35/hr DOE 6 month contract Start Immediately We are seeking a Social Worker/Care Manager II to join our team in San Mateo, CA.

The Social Worker/Care Manager II provides care management to Santa Mateo County older adults and adults with disabilities who are at imminent risk of nursing home placement or are current nursing home patients, transitioning them to live safely in the community.

The Social Worker/Care Manager II provides case management services for a caseload of 15-30 moderate and intensive Community Living Connection (CLC) clients. The Care Manager II plans, monitors services and interventions ensuring provisions of quality care of clients with multiple medical and psychosocial problems. Additional key duties and responsibilities of the position include, but may not be limited to the following: Job Duties: * Conducting comprehensive assessments and re-assessments of client psychosocial, physical and mental health, environmental and spiritual needs to develop the client Community Living Plan.* Conducting on-going client home, acute hospital and skilled nursing facility visits to review and modify their Community Living Plan.* Providing crisis intervention, advocacy, problem solving and therapeutic interventions services to clients.* Documenting and updating progress notes of all case management activity within 24-48 hours.* Assessing client level of care needs by evaluating client ability to live safely in the community and coordinating appropriate placement if needed.* Acting as liaison between the client, caregiver, physicians and community service providers to update, arrange and monitor client care-based program operation guidelines.* Attending and participating in program administrative and clinical meetings.* Attending continuing education classes and/or in-service training related to case management, gerontology, family and community systems and other areas relevant to the aging population as required.

Requirements: * W. (Master?s in Social Work) degree.* 1 year of experience resolving medical and psychosocial problems for adults living with disabilities.* 1 year of case management skills in the community health care delivery system.* Exceptional communication and presentation skills.* Detail oriented with good problem-solving skills and the ability to prioritize multiple tasks.* Proficient computer literacy skills.

Desired Qualifications: * 1 year of experience working with individuals with mental and/or behavioral health diagnoses and substance abuse disorders.* Licensed Clinical Social Worker TargetCW is an equal opportunity employer.
