AppleOne: Bilingual Community Outreach Workers –

This position will be a combination of remote and in-field work.

Outreach Team members will be tasked with not only calling patients to schedule 2nd doses, but also helping with periodic CT/CI (Contact Tracing/Case Investigation) responsibilities.

In-field work will require travel to various locations in Sonoma County to canvass and support vaccination outreach events at pop-up/mobile and fixed vaccination sites.

Field work will also include community outreach (i.E.

door-door outreach; informing individuals about current vaccination opportunities) at schools and businesses.

This position reports to the Outreach Team Coordinator and Vaccines Unit?s Planning Chief.

This position requires professional customer service with an empathetic understanding of community-specific health barriers.

It will also require consistent communication between the Outreach Team members, Coordinator, and Planning Chief.

Dress code will be business casual/casual, depending on in-field or remote work.

Required technical skills include: Google Suite (i.E.

Docs and Spreadsheets), Microsoft Office, also require consistent communication between the Outreach Team members, Coordinator, and Planning Chief.

Required soft skills include efficient time management and transparent, timely communication.

An understanding of public health is key.Interact with Sonoma County communities to educate residents about COVID-19 vaccines during tabling events and community canvassingProvide patient navigation to patients during COVID-19 vaccine clinicsCoordinate with community-based organizations to address health equity barriersWork with an engaging team of Outreach CHW IIs to gather and assemble high volumes of COVID-19 Safety Kits that will be distributed to community partners in order to support county-wide health equity efforts.AppleOne is proud to be an Equal Opportunity Employer.
