Community Resource Worker

Child Guidance Resource Centers

This is a Full-time position in Lansdowne, PA posted August 27, 2021.

The Adult Services’ Supportive Housing Aging in Place (SHAIP) Program’s Community Resource Worker coordinates Wellness Program components, including Fitness, Nutrition, Mental Health support, Meals Program, Home Healthcare, Cultural Arts Programming, socialization, & other programming specific to the residential community.  The Community Resource Worker assesses counsels, recommends, and / or resolves resident issues impacting resident’s personal life, health and well-being, and refers residents to suitable community resources, resources, and Entitlement Programs as appropriate.  

Some responsibilities include:

  • Collaborate with the leadership team to establish linkage with other appropriate service organizations for the older adult.
  • Participate in the coordination of meetings relating to the residents, including CO-MAP and other required individual related meeting to access, provide or coordinate mental health, physical health, and D & A services for this resident group.
  • Participate in the pre-admission assessment as well admission assessment to assist in determining appropriateness for the individual as well as to plan and orientate the resident into the program.
  • Develop, monitor, evaluate and revise the Master Goal Plan with and for the residents within this group.
  • Provide 4-6 hours each of individual counseling, group counseling and psycho-educational groups per week.
  • Provide supportive collaboration to Mental Health Workers, which includes but is not limited to, co-facilitating activities evenings/weekends as determined by the schedule.
