Nurse Practitioner/Physician Assistant At Integra Accountable Care Organization We are seeking an experienced nurse practitioner or physician assistant to provide care management for our geriatric patients in our Accountable Care Organization with the most complex medical needs.
About the Opportunities Home visits and tele-video visits to manage patients with chronic diseases, such as CHF, COPD, DM, upper respiratory infections, urinary tract infections and cellulitis Perform assessments, prescribing and medication reconciliation Approximately four visits/day within 45 miles of Providence/6 telephonic visits or combination of both Monday Friday; 8:00am 4:30pm schedule No evenings, weekends or call coverage Competitive salary plus rich benefits, including health, malpractice and retirement About Care New England Health Care System Fully integrated health system serving Providence, Rhode Island and southeastern Massachusetts Services include primary care, surgery, cardiovascular care, oncology, psychiatry, behavioral health, newborn pediatrics and womens health services Affiliated with Kent Hospital, Warwick, and with Women & Infants and Butler Hospitals in Providence About the Region Providence, the capital and largest city in Rhode Island, offers abundant cultural, recreational and modern city amenities Easy access to other major cities: one hour from Boston, Hartford, New Haven and three hours from New York City.
