The Social Worker coordinates the assistance necessary to meet patient specific needs relative to placement, basic living resources, home health resources, therapeutic counseling, and post hospital support in order to optimize outcomes.

The Social Worker works with the Care Coordinator and other patient care team members.

The Social Worker serves as an advocate to ensure that the patient’s physical, psychological, and social needs are identified and fulfilled on a timely basis and that rights are respected and addressed.Experience Requirements:2 Years Clinical Case Management Required Or 2 Years Related Position Experience RequiredEducation Required:Master’s Degree Social Work RequiredLicensure/Certification Requirements:Licensed Clinical Social Worker Or Certified Case Manager Preferred And Maintain Vehicle Operator Insurability RequiredHours of Work:M-F 8-430Weekend Requirements:No WeekendsCall Requirements:None
