Overview:Utilizing the Nursing Process, coordinates and implements care of patients within the cardiac rehab program under the direction of the Nurse Manager/Department Manager.
Performs work of considerable difficulty.
Represents nursing within Trinity Medical Center and works to improve the health of the communities we serve.Responsibilities:Patient Evaluation and care planning* Performs initial evaluation of patients and develops treatment plan.* Collects initial preprogram outcomes information.* Enters patient information into LSI monitoring system.Goal evaluation and care progress* Interviews patient for progress toward initial treatment plan goals.* Makes appropriate care recommendations to referring MD.* 30 day treatment plans prepared for physician review and signature.Patient safety and education* Surveillance of patients during exercise.* Monitors patients heart rhythms and signs and symptoms.* Assesses patients having abnormal response to activity.* Educates patients on heart healthy living.Qualifications:Education:Must be a graduate of accredited School of Nursing.Previous acute care experience preferred.Experience:Licensed RN demonstrating clinical efficiency2 years of nursing in unit with CV patients or ICU experience.License(s)/Certification(s):Valid driver’s license when driving any vehicle for work-related reasons.
Current Registered Professional Nurse licensure in the State of Illinois and Iowa.
Must maintain current ACLS and CPR Certification.Specialized degree or certification in cardiovascular clinical area (i.E.
CCRN, ACSM Exercise Specialist, CCRP).Knowledge/Skills/Abilities:Writes, reads, comprehends and speaks fluent EnglishBasic computer skills with Word, spreadsheets, e mail, web browserPatient focused caregiverCurrent knowledge of cardiovascular nursing and medicationsWorks well in team settingOther:Competent in use of Epic and LSI monitoring systems within 3 months.#indNURSE
