POSITION: Social WorkerSTATUS: Non ExemptPOSITION SUMMARYResponsible for social worker responsibilities including discharge planning, and social services planning.
Occasionally consults by phone outside of regular working hours for discharge planning and social services issues.MINIMUM QUALIFICATIONSMust have basic understanding of clinical assessment for discharge planning needs.
Must be knowledgeable of community vendors and providers for post-acute discharge services.
Must have strong organizational and teaching skills.
Must be able to communicate effectively and professionally in writing, verbally in person, and over the telephone.
Must have excellent interpersonal skills to effectively communicate with all levels of hospital staff, patients, and patient family/visitors.
Must have the ability to prioritize, to be decisive, tactful and flexible.
Must have strong problem solving and analytical skills.
Must be proficient in personal computer applications with typing skills.EDUCATION AND EXPERIENCERequired: Minimum 2 year clinical experiencePreferred: 1 year discharge planning experience in a medical settingLICENSE AND CERTIFICATIONRequired: Licensed Master Social WorkerPreferred: Licensed Clinical Social WorkerREPORTING RELATIONSHIPReports to: Director of Quality/Utilization Mgt
