This community hospital, in the heart of Los Angeles, is actively interviewing RN Case Managers to support its Care Transition process.

This hospital, well-known for its dedication and long service to the communities of the LA Area, has recently undergone an expansion to fulfill its missions.

The Care Transitions Nurse will get to work with operations leaders from many different functions of this integrated health care system, supporting each.

Reporting to the RN Manager of Care Management, the Care Transitions Nurse Case Manager will work with a large, multidisciplinary team of Inpatient Certified Case Managers (CCM) and Ambulatory / Outpatient Case Managers to evaluate the hospitals resource use and patient care.

The Case Manager will also get to work alongside Utilization and Care Management specialists from the hospital and health plans to identify patients’ needs as they transition back to home-based care.

The Care Transitions Case Manager will play an important part in patient care, and will have the opportunity to work with an outstanding team.

This person will perform follow-up communications with patients after they’ve been discharged to confirm they have everything needed to continue a safe recovery at home.

The Case Manager will help in scheduling and reminding the patient about follow-up appointments.

Working closely with the Manager of Care Management, Post-Acute Services Staff, Social Workers, and the rest of the Case Management team, the Care Transitions Case Manager will serve as a liaison and advocate for patient care.
