· Communicates with courtesy when interacting with internal or external customers—in person or on the telephone.

· Utilizes time effectively and accomplishes duties in necessary timeframe with minimal supervision.

· Assists staff in the delivery of patient care: assessment, diagnosis, identification of outcomes, planning, implementation, and evaluation.

· Demonstrates knowledge of hospital, division, and unit Standards of Care and Practice by serving as a resource person to nurses and physicians during assigned shift.

· Mentors and coaches team members to successfully manage patient needs.

· Assists staff in effective time utilization.

FINANCE:
· Considers factors related to safety, effectiveness, cost and impact on practice in the delivery of nursing services.

· Prepares daily shift assignments, which distributes personnel equitably and appropriately to match the needs of patients with skill of nursing staff.

· Adheres to staffing matrix and operational budgetary benchmarks.

OUTCOMES:
· Educates staff as to their affirmative duty to report incidents, the necessity of such reporting and proper mechanisms of incident reporting.

· Assures incident reports have been filed on incidents employees may be aware of or discovered through chart review.

· Assures that reportable events occurring within the department are properly reported to Risk Management within the time required by law.

· Assists Risk Management Department in identifying risks, analyzing cause/effects of incidents and in the investigation of incidents as may be required for legal purposes.

· Works with Risk Management Department in developing corrective action plans addressing trends or categories or risks/incidents.

· Implements actions, as appropriate, recommended by Performance Improvement activities and hospital-wide Quality Improvement Council (QIC).

· Ensures compliance with core measure/HCAHPS composite benchmarks.

· Documentation meets criteria for core measure, charges, PI, and all quality/regulatory standards as measured by chart audits.

· Assists in completing, monitoring, and evaluation of unit-based Performance Improvement activities.

· Promotes staff development through patient care assignments, acting as a clinical resource and role model, identifying educational needs and ensuring staff attendance at educational in-services.

· Encourages and supports front-line decision making.

· Provides staff in-services on a regular basis.

Assessment: Collection of comprehensive data pertinent to the patient’s health/situation and unit specific data for process improvement initiatives including: initial patient assessment within expected timeframe, initiates consults as indicated from database collection, identifies patient/family teaching and discharge planning needs.

Diagnosis : Analyzes the assessment data to determine expected outcomes and plan of care.

Outcomes Identification: Identifies expected outcomes for the plan of care individualized to the patient or situation and for unit specific PI outcomes.

Participation in identification of unit specific PI outcomes.

Planning: The patient’s plan of care is developed, individualized and prioritized daily in collaboration with the patient, family and health team members that is sensitive to the patient’s cultural and spiritual needs and to attain expected outcomes.

Implementation: Implements the identified plans for patients and unit specific process improvement with appropriate nursing interventions and physician’s orders in an accurate, timely manner and documents intervention according to hospital policy.

Coordinates implementation of the plan of care through leadership and delegation appropriate to skill level including but not limited to: LPNs, NTs, CNAs, Unit Secretaries, RTs, PTs.

Educates patient/family based on assessed learning needs.

Evaluation: Evaluates progress toward attainment of expected patient outcomes and unit specific process improvement outcomes daily with patient, family and health team members.

Practice:
· Maintains safe practice by demonstrating unit specific clinical competency and demonstrating compliance with all regulatory standards.

· Demonstrates safe practice in the administration of medications.

· Ensures safe and effective handoff communications and documentation of such between care givers.

· Demonstrates sensitivity to pain management by alleviating and reducing pain to level acceptable to patient.

· Integrates ethical decision making into daily nursing practice.

· Integrates research findings and Evidence Based Practice into practice.

· Acts as a patient advocate.

· Demonstrates critical thinking skills appropriate to level of experience.

· Integrates population specific standards when planning care.

GROWTH:
