The Health Educator will provide increased access to chronic disease screening, disease management coaching, and the opportunity to lower the impact of chronic disease through the support of lifestyle modification and medication management.

The Health Educator will work within a collaborative care team and be supported by both the Medical Director and the Nursing Director.

The Health Educator may also coordinate, delegate and/or monitor tasks assigned to other members of a care team, such as care coordinators or case managers, with the goals of connecting clients to impactful services, within or external to Western, and improving the client attendance rate.*This is a grant-funded position therefore employment is dependent on and subject to the duration and timeline of the grant.ESSENTIAL DUTIES AND RESPONSIBILITIES+ Identifies those individuals among Western clients who are vulnerable to developing or increasing chronic disease risk;+ Provides health and wellness education to identified individuals and occasionally to groups;+ Plans and implements interventions specifically designed to address individuals’ health goals;+ Evaluates and designs evidence-based and culturally appropriate health education information and materials;+ Creates person-centered care plans and individual wellness plans;+ Assists clients build family and friend supports, as directed by the client, to help clients achieve their wellness goals;+ Collects and interprets patient data and evaluates progress toward individualized patient goals; Monitors health screening data and assists clients to create and update a self-management wellness program;+ Consults with providers and staff regarding related health education and primary care services; Collaborates with primary and specialty care offices in meeting clients’ health care goals and in collecting/documenting preventive screening results;+ Documents results of coaching, monitoring, and wellness planning in the electronic health record; accurately and completely records data, with a specific focus on CCBHC data requirements;+ Maintains professional standards to the community and represents WMMHC appropriately at all times+ Other duties as assigned.SKILLS AND KNOWLEDGE+ Demonstrates knowledge of and use of electronic health records (EHR) and appropriate documentation within the electronic system in a timely manner+ Understands and practices person
– and family-centered care+ Skilled with motivational interviewing and coaching techniques+ Understands and incorporates the objectives and goals of CCBHC status+ Ability to perform, record, and interpret vital signs+ Knowledgeable about medications and the effects of psychotropic medications on physical health+ Comprehensive understanding of privacy and confidentiality requirements+ Knowledgeable in Preventive Health Screening+ Excellent written and verbal communication skills; excellent active listening skills+ Strong customer service skills+ Ability to work independently and as part of an interdisciplinary team+ Ability to prioritize workload and assignments with client outcomes in mind; strong critical thinking and problem solving skillsEDUCATION AND EXPERIENCE+ Current RN license in good standing with the Board of Nursing+ Experience in health education, population health management, or behavioral healthMINIMUM QUALIFICATIONS+ Completion of CPI (Crisis Prevention Intervention), CPR and First Aid training within 90 days of employment+ Ability to pass background check and driver’s license check upon offer of employment.+ Provide proof of auto liability insurance coverage per Western’s policies.
