Resource Nurse Wound Care

Bridgemoor Transitional Care

This is a Full-time position in Pasadena, TX posted December 21, 2021.

The Resource Nurse Wound Care conducts the nursing process, assessment, planning, implementation, and evaluation under the scope of the State’s Nurse Practice Act licensure to include resource utilization and care intervention.

Job Requirements:

  • Valid RN or LVN with current state licensure
  • A minimum of 2 years nursing experience in a fast-paced patient care setting required
  • Wound Care Certification (WOCN) preferred, not required.
  • Experience in a post-acute setting preferred
  • Proficient with Microsoft Office Suite
  • Current CPR Certification from the American Heart Association

Position Reports to Chief Nursing Officer

Overall Responsibilities:

  • Responsible for initiating and completing wound care for all patients.
  • Responsible for completing and documenting wound care assessments in Swift.
  • Responsible for rounding with the wound care doctor, initiate wound care plans, be responsible for weekly documentation of the wound, and ensure all wound dressing changes are completed daily.
  • Responsible for care interventions and interdisciplinary communication to enhance patient and family satisfaction, adherence to properties clinical systems and regulatory compliance for the property
  • Actively participate in QAPI programs and weekly IDT meeting
  • Auditing charts for documentation completion
  • Performing venipuncture for lab draws and including appropriate specimen labeling and obtaining laboratory results via inhouse lab
  • Notify physicians of critical labs while ensuring proper documentation in the EHR
  • Will assist with direct patient care up to and including a complete patient assignment
  • Performs new admission assessments, receives, and verifies new orders and treatments and enters the data into the electronic medical record
  • Completes required documentation in the electronic medical record in compliance with BTC policy and state and/or federal regulations
  • Communicates with physicians, patients, and/or responsible parties regarding changes in patientsÂ’ conditions, diagnostic test results, etc.
  • Collects, reviews trending and analyzes patient data
  • Assists with admissions and discharges of patients to ensure a smooth transition
  • Maintains regular and ongoing communication with BTCÂ’s medical director and other medical staff to ensure each patient receives appropriate care and services
  • Serves as an educational resource for patients and their families on health-related issues
  • Creates and maintains a positive work environment as reflected in staff satisfaction
  • Responsible for quality assurance and assessment, infection control, and staff development as determined by the Chief Nursing Officer
  • Complete the Daily List on a daily basis
  • Make rounds on the patient for which you are responsible daily
  • Answer call lights in a timely manner, be alert to patients comforts and needs, and answer their requests promptly
  • Safely utilize equipment. Report any equipment needing service or repair immediately
  • Complete assigned training on time and/or attend training classes when scheduled
  • Assist in ensuring nursing personnel staffing schedules are prepared and posted to ensure appropriate staffing levels are maintained.
  • Assure attending physicians are given support by nursing personnel.
  • Other duties as assigned

Physical Requirements:

Have the ability to safely perform movements such as pushing, pulling, lifting, bending, kneeling, reaching, and lifting up to 50 pounds with or without reasonable accommodation.
