Ultimate Home Health Services is provides in home health services to clients through our case managers.
Our case managers must be licensed RNs or Social Workers.
The Quality Assurance Coordinator works closely with the Director of case management services and the Administrator to ensure the Quality Improvement plan is achieved.
Review agency Policy and Procedure manual to ensure compliance with EPD waiver regulations, all applicable State and Federal regulations Qualifications: a) Have a current appropriate licensure and have Bachelor?s degree in Nursing or social work and have two (2) years of experience working with the elderly or individuals with physical disabilities; or b) Have a current license as a Registered Nurse (RN) and have a Diploma or an Associate degree in nursing and at least three (3) years of experience working with the elderly and individuals with physical disabilities.
Reports to: The Director of Case Management Services (DCMS) Responsibilities of the Quality Assurance Coordinator * Work with the DCMS to implement the Quality Improvement Plan* Participates in the orientation process for new case managers.* Assures compliance with all state, and federal laws regarding licensure and staff certification.* Ensures proper maintenance of case management records as prescribed by, and in compliance with state and federal laws.* Ensures that case managers coordinate client care as PCSP warrants.* Assist with educational programs for all field staff including In-service program* Stays informed regarding changes and updates with the EPD waiver program.* Understands the business of case management services as established by the agency and DHCF.* Member of the governing body* Member of the Utilization Review Committee (Case management records review}* Resolve complaints with client, family and/ or authorized representative.* Makes quality assurance visits on field staff* Vaccine required and confirming of negative covid test.
