Shift: Days, 8:00am-4:30pm

Job Summary:
As Thome PACE Medical Director/Primary Care Physician, this position will provide direction to Thome PACE related to the medical delivery of care by providers and ensure the delivery of quality health care services.

Supports and directs Thome PACE medically-related committee work.

As Primary Care Physician, you are responsible for providing direct primary medical care to all participants of the program.

As the Medical Director/Primary Care Physician, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.

The Medical Director is responsible for Physician and Nurse Practitioner oversight.

Primary Responsibilities

Medical Director
Assumes overall accountability and responsibility for the medical care of participants at Thome PACE.

Assumes overall accountability and responsibility for quality improvement at Thome PACE.

Provides leadership and medical expertise in the development of medical policies, procedures and guidelines.

Responsible for the development of Thome PACE clinical standards and medical practice guidelines and protocols.

Maintains medical policies / standards and protocols, which are reviewed annually by Medical Advisory Committee.

Chairs the Medical Advisory Committee and Co-Leads the Quality Improvement Committee.

Services as member of the Ethics, Utilization Review, and Pharmacy Review Committees and provides input regarding medical care as needed.

Maintains an active role with the other PACE physicians and medical directors.

Make Total Quality Systems (TQS) a part of everyday operations (implement TQS process as approved).

Responsible for delivery of care and clinical outcomes.

Provides medical guidance and supervision of medical services activities.

Provides oversight of the QAPI Plan.

Reviews all quality of care issues and oversees the development and implementation of quality of care corrective action plans.

Participates in the oversight, training and education of the interdisciplinary team.

Coordinates performance appraisal of the Primary Care Physician and NPs.

Develops educational and other programs to build the skills of participating providers.

In conjunction with Contract designee manages all communication with the provider network.

Represents Thome PACE to external agencies, professional groups and regulatory agencies and organizations as required.

Demonstrates necessary skills and knowledge as outlined in position-specific competency requirements.

Primary Care Physician
Performs comprehensive history and physical on new referrals.

Performs interval history and physicals on established participants completed semi-annually or as indicated.

Assists in the development, implementation and evaluation of the Plan of Care.

Evaluates and treats participants with episodic illnesses.

Refers participants to medical specialists as indicated.

Oversees care of participants cross all settings.

Regulates and adjusts participant’s medications as needed.

Educate participants and families on preventive health maintenance of chronic medical conditions, advanced directives and end of life care.

Actively participates in family conferences.

Coordination of 24-hour care delivery.

Documents participant changes appropriately in the medical record and communicates participant changes to team members.

Participates as a member of the Interdisciplinary Team.

Participates in PCP on-call rotation.

Staff Development
Attend and participate in workshops, seminars, etc., to keep abreast of current changes in the health care and housing fields, as well as to maintain a professional status.

Participant Rights
Ensure that participant’s rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to a prompt response to complaints, are maintained at all times.

Mission Compliance
Serve as an advocate of the facility mission and philosophy of care to participants, family members, visitors, etc.

Respect and honor the life which each participant has lived.

Participate in problem solving in a manner that reflects the facility mission and philosophy of care.

Required Qualifications
Must possess, as a minimum, an M.D.

or DO with current state of Michigan license.

DEA registration and the ability to obtain and maintain staff privileges at Thome PACE contracted agencies.

Board certified in Internal Medicine or Family Practice with advanced training and certification in geriatrics, hospice and palliative medicine preferred.

Immunizations: Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.

Act within the scope of his or her authority to practice.

Meet a standardized set of competencies for the specific position description established by Thome PACE and approved by CMS before working independently and annually.

Experience
Experience working in a managed care environment and working with peers and other health providers to resolve utilization, quality management, performance improvement, pharmacy and therapeutics, peer review, credentialing, and physician leadership issues.

Minimum 3 years of experience in a lead administrative role.

Minimum of 1-year primary care experience with the frail and elderly or long-term care population.

Skills and Knowledge
Must be self-directed, flexible and committed to the team concept.

Able to read, write, speak, and understand the English language.

Possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.

Function independently and have flexibility, personal integrity, and the ability to work effectively with older adults, personnel, and support organizations
