Here is some of what you can expect from being aSocial Worker IIwith us:Perform psychosocial assessments for newly diagnosed and returning oncology patients, and develop and implement appropriate interventions that support the patients identified needsProvide counseling support around impact of diagnosis, prognosis and treatment interventions Participate in daily interdisciplinary care team rounds to identify ongoing patient needsPartner with RN Case Manager, to ensure post-acute medical needs are appropriate and metRespond timely to referrals from RN CM or other staff when risk factors for social determinants of health are identifiedProvide expertise in post-acute services, benefits, coverage and community resources and assist with referral/applications when applicableIdentify and address barriers to discharge and recovery, including financial, emotional, psychosocial, medical and spiritualMaintain effective communication between members of the care team, the patient and familyCoordinate and facilitate family conferences with providers and members of care teamSupport patient, caregivers and care team with conversations around advanced directives, goals of care and end of life careRefer to and collaborate with inpatient palliative care team when appropriateEngage in community outreach and program development to support patient and caregiver needsProvide expertise and support in ethics casesContinually seek to further knowledge of Oncology Social Work profession through continued education and participation in professional organizations What qualifications you will need:Licensed Social Worker (LSW) in the state of Colorado required.Ideal candidate is working toward LCSW licensure.Master of Social Work (MSW) from an accredited school of Social Work required.BLS Health Care provider, required.Healthcare social work experience required, experience with oncology patients preferred.Understanding of impact of acute/chronic illness on patients/families.

Case management experience preferred as well as knowledge of inpatient, community and post-acute resources.

Basic social worker skills including but not limited to empathy, sensitivity to cultrual and ethnic diversity, advocacy, communication and crisis inervention.Presbyterian St.
