The Infectious Disease Pharmacist (ID Pharmacist) is responsible for oversight, strategic planning, and optimization of antimicrobial stewardship program (ASP) throughout the Crozer Health system with a primary focus on CCMC.
The Infectious Disease pharmacist will perform daily monitoring of antibiotics to improve patient outcomes by promoting appropriate usage and minimizing adverse drug events, antimicrobial resistance and costs.
The ID pharmacist will serve as the expert clinical resource for the pharmacy staff and the health care professionals within the organization.
ID pharmacist will be responsible for developing and implementing evidence-based guidelines and protocols.
ID pharmacist will perform antimicrobial formulary review, medication use evaluations and quarterly antimicrobial stewardship reports for the Antimicrobial Stewardship Program committee and P&T committee.
