Description: HCR ManorCare provides a range of services, including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy.The Patient Care Coordiantor will be focused on continuous care oversight and scheduling. Qualifications Minimum of one year experience in providing medical, oncology, home health care, or hospice nursing preferred, may be specified by state. Previous management or supervisory experience preferred. Educational Requirements Currently licensed as a Registered Nurse (RN) in good standing with the State Board in which he/she practices.
